According to WFMZ, the man attempted to use a forged prescription for oxycodone at the CVS Pharmacy, located on 212 Pottsville Street, North Manheim Township, near Schuylkill Haven on January 3rd, 2016 around 1:15pm.
Anyone with information or can identify the man pictured in the surveillance photo is asked to contact Pennsylvania State Police at the Schuylkill Haven Barracks at (570) 739-1330.
